Thank you for choosing the Marion L. Shepard Cancer Center. Our goal is to deliver state-of-the-art medical evaluation and treatment. We want to make your visit to our clinic as comfortable and pleasant as possible.
To help in this process, please complete the necessary forms prior to your visit.
On the date of your first visit, please:
- Arrive 30 minutes prior to your appointment
- Bring all completed forms. (History form, Privacy Notice form)
- Bring your insurance card(s). If you do not have insurance but are working, please bring in your last pay stub. If you are not working, please bring in a copy of your Social Security check, disability check or any documentation that can support the amount of income you have.
- Bring all of your medications.
- You can expect your first visit to take approximately 1 – 1 ½ hours.
